That particular trope is also probably a consequence of Sod's Law.. 'The more there is to go wrong, the more there is to go wrong'. The bigger an organisation, the more complex its systems will be. Couple that with each user being a 'part' of the system and the IT project 'good enough' standard, and it would be a miracle if any large system worked without an error, hence every program - government or otherwise - sending out updates to correct the latest flaw. Also, when government projects fail they are always well publicised because they affect millions of users. The (probably) hundreds that don't fail are not newsworthy.
