If you specify "apt-get upgrade", then packages will only be upgraded if they do not require any new packages to be installed. If this happens you will get an "xyz package will not be upgraded" message. Sometimes, you need "apt-get dist-upgrade" to ensure that you get the latest version of your packages; this will install new required packages too if necessary.
