They are commonly harvested from the address books of people who run windows and let their windows machines be infected with trojans, etc.

Then there is the "human" harvesters - people who are paid to find and subscribe to mailing lists to harvest the email address of people who post to the lists.

You can't escape this harvesting...

I guessed it was some form of harvesting. The one that puzzled me was a few years ago when my Dad's account once had spam. He never actually used it, the only reason it existed was because I registered an ISP account for him to display his art work on his webspace - non commercial so the email account was never on it.
