Are you definitely using the HDMI port on the back of your PC, a HDMI cable and the HDMI port on the monitor itself? xrandr doesn't think so, and it would go a long way to explaining what's going on here. It looks like at least one of those three pieces has accidentally got connected to the wrong something. No wonder you're getting weird results! It would really help to know which exact motherboard you're using as well so I can check the physical layout of the connectors. Even with the similarities between HDMI and DP it would be pretty hard to manage to plug things in incorrectly but you never know.
