Looked at RT - good for Perl hackers - but it isn't exactly easy to install, or set up, but does everything with sufficient push. Best get someone who has done it before if going this route.
Quite a lot of third party offerings around, software as a service, which works well for helpdesks as they continue to work when everything else is broken.
