Have you actually got an entry for /dev/dvd in /dev?
Not on my desktop (where I am at the moment) - I have links from dvd2,
dvd3, dvdrw2, also cdrom2 ditto, and also scd0, scd1 which are not
links. I'll have a look in my laptop. But why do other dvds mount and
offer a window of options?

Maybe it's mplayer just looking at /dev/dvd and bypassing fstab. I believe that mplayer accesses the DVD drive directly rather than mounting the disc and reading the files.

Other discs would probably automount as there should be an entry in fstab pointing probably to something like /dev/scd0.

I'm not sure if it's possible to override where mplayer looks for the DVD in the configuration file.
