If you allow people to direct-dial your VoIP phone or VoIP PBX, then it's up to you to control the source of the incoming calls. If you're getting calls via an ITSP then they may have provision to block malicious callers. But basically, with your own VoIP PBX, if you want to accept calls from anyone, then you'll get calls from anyone, spammers included.
