Typically a maximum number of connections was reached for the general
number of clients or clients from a particular source.

This might be a limit on individual IP addresses or subnets etc. You
also might get problems resulting from tcp wrappers denying access.

There are of course many many many many other possible reasons.
You can send some logs or additional information if you like.
