Fairly easy, cd to the /boot directory
do mkinitrd initrd-2.4.9-21.img 2.4.9-21 (Subsistute the numbers for you kernel
version including the redhat revision numbers.
In lilo.conf make sure there is line that reads initrd=initrd-2.4.9-21.img
So in lilo.conf you should have something like...
image=/boot/vmlinuz-2.4.9-21
label=linux
initrd=/boot/initrd-2.4.9-21.img
read-only
root=/dev/sda1
