In lilo.conf make sure there is line that reads initrd=initrd-2.4.9-21.img

...and there you have it :-) I am pretty sure that I deleted this line,
which now of course makes sense of what I was seeing. With no module loaded
there was no way the system could access the disk to mount the root
partition. Invariably I delete this line and the ramdisk one out of lilo
simply because they aren't used. In this case, of course, it was :-)
