I added: RewriteLog "/var/log/apache2/rewrite.log" RewriteLogLevel 3 to sites-enabled/000-default and restarted apache. rewrite.log is created but browsing around the site doesn't produce any entries. I deleted rewrite.log and moved those options to apache2.conf instead. Again rewrite.log is created but not populated. access.log confirms that the requests are getting to apache. In sites-enabled/000-default I just tried setting RewriteEngine off and deleting rewrite.load from mods-enabled. it's still rewriting!
