Tom,

Our Toshiba has always had a noisy fan but recently it started spinning
up up very often. The cause was blocked vents.
It sounds like the fan is on its was out and in need of replacement.

My Acer Aspire had a similar problem and it was due to blocked vents.  Once I opened it up, the fan itself was fine.  From what I could see, the vents were clean as well.  It was when I took the copper vent out and poked a Q-tip through that a chunk of built-up dust came out.
