Skanlite with sane for scanning and tesseract-ocr or ocropus for ocr.

I've had no problems with tesseract-ocr or ocropus although both are 
command line. I prefer ocropus but both are quality software.

i don't know if kooka (gui for both scan and ocr) is still around. It 
was fiddly but did the job.
