I kind of have something like this already with my SipGate accounts. I
have two geographic numbers, a 01872 (truro) and a 01326 (falmouth) and
any incoming PSTN calls get routed to my sip phones automaticly.  Any
SipGate or friend company based calls to that number get a direct VOIP link.

I guess the main difference here is that i can't register an arbitrary
number, a new number is allocated from a pool of available numbers in
each exchange. And this is limited to SipGate and friend companies not a
internet wide system of lookups as the look ups are internal to SipGate.

Its also free which is good (no registration fee) and i only need to pay
for outgoing calls to a PSTN number, incomming calls from SIP or PSTN
are free.
