Commonest GP IT grumble apart from programs falling over is of the noise
the PC makes in a consulting room.

Long cables for screen, keyboard and mouse.  Cheaper, less complex, faster,
and screen resolution as native (if you see what I mean).

Assuming you have a suitable place to put the PC on the end of
the cables.

I bet wholesale the price difference between silent components
and fanned is pretty small, probably of the order of a couple of
quid per PC.

If someone could guarantee sales of a few hundred it would be
worth doing, paying retail and retrofitting the equipment costs
more than my PC's are worth.
