EDS are giving us, it is rumoured, a proprietary solution for that much.

HOW MUCH? Even if they continued to used Microsoft Outlook, or Novell 
Groupwise, or even another current commericial solution out there, it would 
cost less.

Give them a decent IMAP/LDAP/POP3 solution with something like a Horde IMP 
server farm would (Guessing) way less than that.  The only tricky extras is 
keep things such as patient records confidential.
