Regrettably, several.
One is in the hands of the health authority and is therefore wholly 
unsuitable for this purpose, several others will be for local medial record 
systems.
The PVS system which I am involved in doesn't differentiate on location any 
closer than by country or (US) State so it would only be useful as a first 
layer, and probably having more than one layer is not good.

Even if they don't presumably if we create an authentication
scheme you will want to reuse it in future.

Some of the specialist NNTP hosting companies may have all this
prepackaged by now.
Can you point at any of them or where to find them?

I tend to think in terms of minutes and sumaries rather than an archive
in use for every passing comment.

Okay 48 hour expiry like alt.binaries.* on Demon then ;)
6 month more like.  We move at a slower pace in the health service than most 
people can even balance at.
